To apply for a tattoo please answer the questions from this consultation form. These questions are necessary for me to assess your project fully before proceeding.

All of the information is vital.

Please answer ALL of the questions, and be as SPECIFIC with your descriptions of the tattoo as possible.

If the tattoo you want to get is a copy of someone else's tattoo, artwork or style, I will reject your project. I do not copy other peoples work in any way.

Do not send the answers for people other than yourself. Anyone wanting a tattoo needs to take the time to answer these questions and email them themselves. Third party consultations will be discarded.

1) What is the specific subject matter of your tattoo? Please do not give multiple choice answers. Be specific and precise with what you want. (ex. Specific flower, specific animal, specific portrait, text, etc.)

2) Where will your tattoo be located? The location is important to the design.

3) What size would you like your tattoo to be? Please use measurements not terms like medium.

4) Is there a story that you feel is of vital importance to the understanding of the deeper meaning of the tattoo? Or is there something you would like to say about yourself to help understand who you are ? If so, please give a short description.

5) Are there other tattoos in proximity to your new tattoo, or is this a cover-up tattoo?

6) Have you been tattooed by Gene before or are you continuing/adding to a tattoo that he started? If so, what is the tattoo?

7) What is your skin tone? (ex. Fair, light, tanned, medium, dark, etc...)

10) How would you describe your body type? (Athletic, skinny, muscular, large, curvy, I live in the gym bro!, etc...)

14) Do you have specific dates that you are planning on coming to New York? Or can you indicate a time frame that you are available? weekdays/weekends?

Please do not book a trip for the tattoo until you have been given instructions for how to book your appointment and are confirmed.

15) If you are from out of town, how often would you be able to return for appointments if your project requires multiple sessions? Are you available for multiple sessions? Many tattoos require at least 2 sessions to complete.
